WAIVER OF LIABILITY & RELEASE

Football Mega Camp — July 19, 2026 · Webber International University

PLEASE READ THIS DOCUMENT CAREFULLY BEFORE SIGNING. This Waiver of Liability and Release ("Waiver") is entered into by the parent or legal guardian ("Participant's Representative") of the minor athlete named in this registration form ("Participant"), and applies to all activities associated with the Football Mega Camp to be held on July 19, 2026, at Webber International University, Babson Park, Florida ("Event").

ACKNOWLEDGMENT OF RISK: I, the undersigned parent/guardian, acknowledge and fully understand that participation in football camps, athletic training, and related activities involves inherent risks of physical injury, including but not limited to: sprains, strains, fractures, contusions, concussions, heat-related illness, cardiovascular events, and other injuries that may occur despite all reasonable safety precautions. These risks exist regardless of the skill level of the participant or the precautions taken by organizers, coaches, and staff.

RELEASE OF LIABILITY: In consideration of the Participant's enrollment in and participation in the Event, I hereby voluntarily release, waive, discharge, and hold harmless the following parties and their respective officers, directors, employees, agents, volunteers, representatives, sponsors, and assigns from any and all liability, claims, demands, damages, costs, expenses, and causes of action arising out of or in connection with the Participant's attendance at or participation in the Event, whether caused by negligence or otherwise:

Speed Plus Academy, Inc. (SPA) · Speed Plus Sports (SPS) · Felton Athletic Management (FAM) · Webber International University (WIU) · All respective officers, directors, employees, coaches, staff, volunteers, and facility personnel of the foregoing organizations

MEDICAL AUTHORIZATION: I certify that the Participant is physically fit and able to participate in the athletic activities described herein. I authorize camp personnel, coaches, and staff to obtain emergency medical treatment for the Participant if I cannot be reached and such treatment is deemed necessary by medical professionals. I understand that I am responsible for all medical costs incurred on behalf of the Participant.

PHOTO / MEDIA CONSENT (Optional): By checking the media consent box below, I grant permission to Speed Plus Sports (SPS), Speed Plus Academy, Inc. (SPA), Felton Athletic Management (FAM), and associated organizations to photograph, film, and/or record the Participant during the Event and to use such images, video, and recordings in promotional materials, social media, websites, and ministry content, without compensation.

INDEMNIFICATION: I agree to indemnify and hold harmless the above-named parties from any claims made by or on behalf of the Participant arising from participation in the Event.

SEVERABILITY: If any provision of this Waiver is found to be unenforceable, the remainder shall continue in full force and effect. This Waiver shall be governed by the laws of the State of Florida.

VOLUNTARY AGREEMENT: I have had sufficient opportunity to read this Waiver, to ask questions, and to obtain legal advice. I am signing this Waiver freely and voluntarily. I understand that by signing, I am giving up substantial legal rights.
